Waiver and Release of Liability

I __________________________, acknowledge and agree to the following terms and conditions as a participant (or as the parent/legal guardian of a minor participant) in any program offered by Pro Swim Academy:

1. Assumption of Risk:

I understand that participation in swimming and aquatic activities involves inherent risks, including but not limited to slips, falls, drowning, and other injuries. I voluntarily assume all risks associated with participation in any program or activity offered by Pro Swim Academy.

2. Release of Liability:

I hereby release, discharge, and hold harmless Pro Swim Academy, its employees, instructors, and affiliates from any and all claims, liabilities, or causes of action arising out of or connected with my (or my child’s) participation in its programs, whether arising from negligence or otherwise.

3. Medical Emergencies:

I give permission to Pro Swim Academy to seek emergency medical treatment in case of injury or illness. I understand that I am responsible for all medical expenses incurred.

4. No Refund Policy:

I understand and agree that all payments made to Pro Swim Academy are non- refundable, regardless of the reason for withdrawal, cancellation, or inability to attend scheduled classes.

5. Make-Up Classes:

In the event of a missed class, a make-up class may be scheduled, subject to availability. All make-up classes must be arranged in advance and completed within the current session period or as specified by Pro Swim Academy. Make-up classes are not guaranteed and are offered as a courtesy, not an entitlement.

6. Photography and Media Release:

I give permission for photographs and videos taken during classes to be used by Pro Swim Academy for promotional purposes. (Optional – you may remove or modify this clause as needed.)

Acknowledgment

I have read and understood the above waiver and agree to the terms and conditions. I understand that this document is legally binding.
